In the Heart Health Program, your heart care doesn’t stop between doctor visits. Our virtual care team works closely with your doctors to give you the tools and confidence to take charge of your own health. We are here to support you, and we will make sure you are not alone on your health journey.

Patient Engagement Specialist (PES)
A Patient Engagement Specialist is your main point of contact. They will help you get started with our programs. Speak with them when you need help or have a question. They will make sure you have everything you need to stay connected with your care team.

Registered Nurse (RN)
A licensed registered nurse will keep a close eye on your health needs and answer medical questions. They will plan with your doctor to make sure you are getting the best care at the right time.

Health Coach (HC)
A Health Coach is a certified professional who is your guide through the program. They will help you build healthy habits like eating well or moving more. They will give you the tools and support you need to reach your goals. The Health Coach helps you stay healthy along your journey.

Medical Assistant (MA)
A medical assistant’s goal is to help you manage your health concerns. They will check in with you often to see how you are doing. They will also help coordinate what you need for your care so you can focus on feeling better.
