Just because you completed the program with your Health Coach does not mean you are alone. Stay connected to your care team.

ACTION PLAN FOR STAYING HEALTHY



Keep Your Routine Going

o    Maintain the healthy habits you built during the program.
o    Remember lifestyle changes take time! You will see benefits when you keep up with healthy habits.



Stay Physically Active

o    Aim for at least 150 minutes of moderate activity each week.



Eat Balanced Nutritious Meals

o    Focus on the right fruits, vegetables, whole grains, lean proteins and healthy fats.
o    Stay hydrated.
o    Limit highly processed foods and added sugars.



Monitor Your Health

o    Check your vitals at home. This includes your blood pressure, weight, heart rate, sugar. 
o    Use the tools or journal to track and notice patterns.
o    Call us or your provider if you notice the numbers are out of range.



Continue Preventive Care

o    Schedule regular checkups and recommended screenings.
o    Keep your medicines and shots up to date.



Reach Out When You Need Support

o    Call us BEFORE you go to the Emergency Department. We will try to get you access to the care you need.

*As always, you should follow the recommendations from your provider and consult them when making any changes.
